School-Age Equipment Needs

School-Aged Equipment Needs

Children are eligible for this program from the start of school to 18 years of age. Younger children with home-based equipment needs do not need a separate referral as these needs will be met by their primary therapists.

If the child is involved with an Occupational or Physiotherapist through a Niagara Children’s Centre program (School-Based Rehabilitation Services, School-Aged Active Rehabilitation) or clinic (Seating and Mobility, Casting and Splinting, Home and Vehicle etc), a separate referral is not required. The parent/legal guardian should contact the therapist directly to request equipment.

Referrals can be made by the HCCSS Care Coordinator, physician/primary care or healthcare provider, or parent/legal guardian.
