The Intake Coordinator responds to and processes all external referrals for Centre as well as facilitates connections for families to other community programs/services as appropriate based on child and family goals, strengths and needs.

Included in the role is interacting with families and referral sources to determine child and family goals, strengths, and needs through exploratory conversations and the use of formal and informal eligibility and triage tools, determination of referral eligibility for Centre services, scheduling of screening/assessment appointments for Centre services, and responding to inquiries related to Centre wait times.

Key responsibilities include:

Exploratory Conversations and Administration of Triage Tools: The Intake Coordinator may conduct an exploratory conversation utilizing a variety of tools based on presenting concerns/needs, including the All About My Baby, All About My Child, All About Me Tools. Based on presenting concerns and age of the child, detailed questions regarding all areas of child development (e.g. communication, motor/mobility, activities of daily living, emotional/sensory regulation etc.) may be discussed with the parent/guardian to determine appropriate next steps/initial clinical pathway.

Initial Family Information and Orientation: Provide families with Centre orientation materials and general “need to know” information. Provides advice regarding and facilitates the most appropriate initial clinical pathway and next steps for the child, based on the information gathered through the exploratory conversation/triage tools.

  • Reporting: Keep managers/clinicians abreast of any issues related to consent/custody or triaging between programs as well as any trends in client and family concerns/needs/referrals by geographic location or issues at any satellite offices involving screenings.
  • Data Entry: Maintain and input accurate information into relevant data bases related to new and internal referrals.
  • Scheduling: In negotiation with the parent and with information acquired via the triage tool, schedule a screening appointment with the relevant screener and provide respective follow up communication while waiting for service. For services without direct scheduling by Intake Coordinators, follows procedures to ensure child is placed on the appropriate service waiting list based on need.
  • Liaising with the community: provide information and answer questions regarding our services, request clarification or additional information from referral sources, inform referral sources if families do not respond to attempts to contact; facilitating supportive connections to other community service providers and/or directing families to other community services as appropriate.
  • Correspondence: provide fax information to physicians regarding status of clients and appropriateness of referrals; provide “unable to contact” and discharge letters to families.

Health and Safety

The Intake Coordinator will comply with the duties assigned to Workers as per Section 28 of the Ontario Occupational Health and Safety Act.

Key Relationships:

The Intake Coordinator is concerned with the effectiveness of all relationships that affect the Centre. The following are particularly significant to the position:

External

  • Families, caregivers, and youth
  • Community partners
  • Community at large
  • Primary Care

Internal

  • Clinical managers and clinical staff

Qualifications and Skills:

The minimum specifications for an incumbent will include:

  • Graduation from a recognized college/university in a child development related field (e.g., ECE/Resource Teacher, CDA; OTA/PTA; Bachelor of Arts in Child Studies)

Knowledge/Skills:

  • A sound understanding of child/youth development across all developmental domains
  • Minimum of 2 years’ experience working with infants/young children with identified needs using a family-centred model of intervention
  • Expertise in child development and familiarity with common pediatric conditions, as well as knowledge of the local service system to support service navigation.
  • Demonstrated ability to consistently maintain professional and respectful communication and collaboration with clients and families, colleagues, managers, and community partners.
  • Knowledge and demonstrated ability to uphold principles of family-centred care, provide exceptional customer service, and utilize a strength-based, anti-racist, anti-ableist, and anti-oppressive approach to service delivery and interactions with stakeholders.
  • Demonstrated initiative and self-management.
  • Computer literacy utilizing MS Office software applications.
  • Ability to utilize developmental screening tools for determination of eligibility and triage decisions.
  • Ability to effectively support families in complex situations.
  • Ability to work with families from a variety of cultures that is inclusive and culturally appropriate.
  • Bilingual at a superior or advanced level of French is an asset.
  • A current criminal reference check for vulnerable populations.
